SKILL 13: MEASURES AND RECORDS THE WEIGHT OF AMBULATORY PATIENT
- Introduce yourself and explain the procedure to the patient.
- Perform hand hygiene
- Verify the patient is wearing nonskid footwear.
- Balance (or zero) scale.
- Walk the patient to the scale.
- Assist the patient to step on the scale.
- Check that the patient is centered on the scale.
- Check that the patient has their arms at their side.
- Ensure the patient is not holding on to anything that would alter the reading of the weight.
- Adjust the weights until the scale is in balance or read analog scale.
- Perform hand hygiene.
- Document weight (plus/minus 2 lbs. or 0.9 kg)
